Important!

It is especially important to update you child's information regularly!  If your child becomes sick or has an accident, we need to be able to get in touch with you immediately!  Also, if school is let out early or if your child has forgotten something important like their eyeglasses, it might be necessary for them to get in touch with you or a grandparent.  This form will help you keep your address and phone numbers up-do-date with the school's information.  Thank you for helping your student and the school out with this matter.
